•La implementación de las vacunaciones en adultos debe contemplarse desde un punto de vista global, lo que se ha dado en definir como “calendario vacunal a lo largo de toda la vida”, sin solución de continuidad tras las vacunaciones infantiles.•Alcanzar altas coberturas de vacunación sistemática es la base para la reducción sostenida de las tasas de morbilidad y mortalidad por enfermedades prevenibles mediante vacunación a lo largo de la vida de todas las personas.•Los principales obstáculos para la vacunación tienen diferentes orígenes; factores logísticos y de financiación, factores relacionados con los profesionales sanitarios y factores relacionados con el propio paciente.•En 2017, la OMS publicó “Estrategias y prácticas mundiales de inmunización sistemática (GRISP): documento complementario al Plan de Acción Mundial sobre Vacunas (GVAP)”, donde recoge las estrategias generales para mejorar las coberturas vacunales.•Las actitudes de los profesionales sanitarios hacia la vacunación se encuentran entre las influencias más importantes en la decisión de vacunar.•En el ámbito de la atención primaria, los sistemas de recordatorios como las llamadas telefónicas, cartas, postales, mensajes de texto, combinación de correo, teléfono o recaptación del paciente pueden ser eficaces para mejorar la proporción de la población objetivo de las inmunizaciones.
