Allergy reconciliation is a key contributor in decreasing morbidity related to adverse drug reactions. Implementation of a standard process for allergy documentation has been shown to increase the accuracy and completeness of allergy documentation in patient charts. This organizational quality improvement study, utilized a retrospective chart review. The project was implemented in a private practice ambulatory surgery center located in the Midwest. A baseline chart audit was conducted prior to implementing the three month pilot test of the standardized process for allergy documentation. A post-intervention chart audit was conducted after the pilot period to evaluate effectiveness of the standard process. The results of the post-intervention chart audit revealed an increased accuracy of allergy documentation after utilizing the standardized process.
