Diferencias en costo efectividad del cuidado paliativo domiciliario frente a la atención convencional hospitalaria para el manejo de enfermedades crónicas en fase terminal

In Colombia, the population dynamic has changed the last years, which modifies the epidemiological characteristics and mortality profile of the population, increasing the incidence and prevalence of chronic diseases. Given this, and the limited capabilities of hospitals, it is essential to determine the differences between the care provided in palliative care programs through home care and in the hospital. Systematic review of several articles, included in levels I and II evidence, including those that describe differences in place of death of the patient, relief of symptoms, patient satisfaction and their caregivers with the services received, number of hospital admissions for any cause and average days of hospitalization for the two treatment alternatives. The 88% of patients who had been implicated to home care had died at home, also 68% of these patients reported that it had experienced decreased intensity of at least 2 symptoms. A difference in satisfaction of 72% and 53% for home care Vs. The hospital management respectively, also only 23% of patients managed at home required at least one hospitalization during handling, with reduced average stay of 3.4 days. The home palliative to treat the chronic patients in terminal phase is not only an alternative to lessen the costs of the health system, but it helps the patient in various aspects such as accompanying family and loved ones, decreasing the risks associated hospitalization, among others, thus providing involving him and his family in the therapeutic process.
