Review Description

Microvascular obstruction is very prevalent in acute coronary syndromes, is not well recognized, and has serious long-term adverse consequences, including ventricular remodeling, scarring, and congestive heart failure. It is readily diagnosable noninvasively by cardiac MRI. Studies as that by Prati et al. suggest that safe, effective, and simple therapy is possible, but needs optimization. Microvascular obstruction is emerging as a new and exciting frontier for the interventionalist, who can eliminate stenosis in large epicardial vessels but will soon be able to open a multitude of obstructed microvessels. In doing so, the interventionalist will become key in not just treating ischemia in myocardial infarction, but treating heart failure by preventing it. © 2005 Wiley-Liss, Inc.
