Sleep disordered breathing (SDB) is prevalent in the pediatric population, particularly in children with special health-care needs (CSHCN). The types of SDB include obstructive sleep apnea syndrome (OSAS), central sleep apnea syndrome (CSAS), nocturnal hypoventilation (NH), and sleep-related hypoxemia. Untreated SDB conditions can lead to systemic and cognitive deficits that may negatively impact baseline function in CSHCN. Noninvasive positive airway pressure (NIPPV or NPPV) or positive airway pressure (PAP) is common mode of therapy for SDB in this population. PAP, although efficacious for treating SDB, is limited by lack of adherence which may negatively impact clinical outcomes. There are barriers and facilitators to PAP therapy in the CSHCN population that similarly impact adherence in children with typical development. However, there are additional factors that are unique to CSHCN. An integrative multidisciplinary approach to addressing these factors is necessary to optimize PAP initiation and sustained use in CSHCN. This chapter will focus on the factors that impact adherence in this patient population and personalized strategies to improve adherence to PAP.
