Chronic pain patients taking opioid medications may present with difficult-to-manage acute pain. Opioid tolerance and opioid-induced hyperalgesia complicate their management. Non-opioid “multimodal” analgesics are useful alternatives since they bypass opioid tolerance. Local or regional nerve block through administration of local anesthetic is particularly valuable since the resulting conduction blockade can prevent transmission of pain signals in the first place. This chapter will focus on the pharmacology of local anesthetics, common methods of administration, and potential side effects/complications of their use.
