Book Chapter Description

The decision to choose between a canal wall-down (CWD) or canal wall-up (CWU) surgery for cholesteatoma presents a medical dilemma. Although extensively discussed in the literature, the topic remains controversial. Both techniques have powerful arguments in their favor, with the main objectives of surgical management being the complete eradication of the disease and prevention of recurrence and improvement of the patient's hearing and quality of life, while minimizing future complications. In most cases, the surgeon's decision is based on their personal experience and patient's characteristics. Despite the ongoing debate, there is a tendency toward performing CWU over CWD surgery. This chapter provides an updated review of both surgical techniques, reconstruction methods, use of intraoperative endoscopes, and clinical outcomes. Additionally, it presents the preferences and opinions of expert otologists on both techniques.
