Article Description
Fractures of the tibial plateau are frequently accompanied by comorbidities (e.g. anterior cruciate ligament, lateral ligaments, meniscus) and a complex involvement of the joint surface. As a rule treatment is carried out operatively with locking plate osteosynthesis. This article describes the case of a 61-year-old female patient who incurred a fracture of the anterior tibial head and the medial femoral condyle with a multiligament rupture as well as a flap rupture of the lateral meniscus after a fall in the domestic environment. The indications for arthroscopic surgery were therefore given; however, because of the danger of a compartment syndrome the intervention had to be prematurely terminated. Subsequently, the patient was successfully treated with a total knee endoprosthesis. Compartment syndrome is a very rare but serious complication in arthroscopy. Fasciotomy is a good invasive treatment option, whereby the indications depend among others on the pressure of the compartment. Precise guidelines with respect to the treatment of compartment syndrome are not yet available. Treatment should primarily be carried out in a patient-oriented manner.
