Endocrine emergencies are rare in emergency medicine but with an increasing incidence. This article focuses on understanding the pathophysiology of endocrine feedback loops in order to improve the recognition and emergency treatment of endocrine emergencies of the thyroid glands, adrenal glands and pituitary glands. Thyroid storm and myxedema coma are life-threatening thyroid emergencies, which need intensive care treatment. Hypertensive crises can occur in Cushing’s disease, pheochromocytoma and hyperaldosteronism and thus need a specific treatment. Adrenal insufficiency is a life-threatening disease, which is often initially overlooked as it only leads to an unspecific deterioration of the general condition. Diabetes insipidus is the most dangerous acute pituitary disease, which in the case of somnolence can rapidly lead to a hypovolemic shock.
