Article Description
Las alteraciones en el estado de conciencia son frecuentes en el paciente oncológico, la mayoría de los cuales presentarán delirium en fase terminal. Dado que no se detecta adecuadamente, se debe sospechar en cualquier alteración del estado de alerta en el paciente oncológico. La Mini-Mental State Examination (MMSE) es muy útil para su detección en Urgencias. Si bien se recomienda un tratamiento basado en la etiología, un tercio de los pacientes presentarán agitación que se aborda hablando al paciente de forma tranquilizadora, en un ambiente silencioso y ocasionalmente con fármacos, de los cuales los antipsicóticos de segunda generación por vía oral son de elección. Se realizará una analítica, un electroencefalograma (EEG) de Urgencia y una IRM (imagen por resonancia) en los días siguientes para identificar la etiología del delirium, que en sólo un tercio de los casos se podrá atribuir a una única causa. Existen múltiples factores que aumentan el riesgo de padecer un delirium que se deben tener en cuenta para evitar su aparición.
