Article Description
La sífilis es la enfermedad producida por Treponema pallidum subespecie pallidum. Se transmite fundamentalmente por vía sexual. Tras el contagio, la bacteria se multiplica localmente formando una úlcera (chancro) y a partir de ahí se produce la diseminación sistémica. Sin tratamiento, la evolución de la sífilis se puede dividir en dos estadios: precoz (< 1 año) y tardío (> 1 año) que a su vez se subdividen en diferentes fases con manifestaciones clínicas específicas. La sífilis primaria y la sífilis secundaria se consideran formas clínicas precoces, mientras que la sífilis terciaria se considera una forma tardía. La invasión del sistema nervioso central por T. pallidum se conoce como neurosífilis y puede ocurrir en cualquier fase. En la historia natural de la sífilis suele existir una fase asintomática que se denomina latencia. Para apoyar el diagnóstico disponemos de técnicas microbiológicas directas e indirectas. En general, las pruebas directas son útiles solo en el estadio precoz. La penicilina sigue siendo el tratamiento de elección de la sífilis en cualquiera de sus fases.
