Article Description
La neumonía nosocomial es la infección del parénquima pulmonar en un paciente ingresado en un hospital al menos durante 48 horas. El factor de riesgo más importante es la necesidad de intubación endotraqueal y la ventilación mecánica. Los pacientes que viven en instituciones sanitarias también pueden desarrollar este tipo de patología, donde la presencia de patógenos multirresistentes como agentes etiológicos está siempre presente y tiene una gran trascendencia a la hora de instaurar el tratamiento. Este debe ser inicialmente empírico para posteriormente desescalar para dirigir la terapia hacia los resultados microbiólogicos obtenidos cuando aparece la sospecha clínica debido a la presencia de fiebre, leucocitosis, expectoración purulenta y deterioro en la oxigenación. La clave del diagnóstico está en la presencia de un infiltrado radiológico nuevo, progresivo y persistente, aunque esto no siempre es fácil en los pacientes más graves.
