Interest of a 3D custom-made implant in the reconstruction of bone defects of the cranial vault

The authors report a case of a patient managed for severe cranial vault depression following combined neurosurgery and radiotherapy. This situation caused major aesthetic discomfort and was potentially dangerous due to the mechanical weakness of the bone flap. The authors had a CAD (computer aided design) silicone elastomer custom-made implant made to fill perfectly the depression. Beforehand, an expansion was performed to cover the implant after removal of the radiated skin. The surgery and post-operative course raised no concerns. After one year of follow-up, the result is very good and the patient very satisfied, proving that this technique certainly has its place in the therapeutic arsenal when faced with a tissue defect of the cranial vault. Les auteurs rapportent un cas clinique d’un patient pris en charge pour une dépression importante de la voute crânienne faisant suite à une association chirugicale et radiothérapie. Cette situation entraînait une gêne esthétique majeure et était potentiellement dangereuse en raison de la faiblesse mécanique du volet osseux. Les auteurs ont réalisé un implant siliconé construit par CAO afin de combler la dépression. Au préalable une expansion avait été réalisée afin de venir couvrir l’implant après exérèse de la peau radiothérapée. L’intervention et les suites se sont déroulées sans problématique. À 1 an de recul, il existe un très bon résultat et une grande satisfaction chez le patient prouvant que cette technique a certainement sa place dans l’arsenal thérapeutique face à un défaut tissulaire de la voûte crânienne.
