Review Description
Acute heart failure (AHF) affects nearly every Canadian with heart failure (HF) at least once. Despite several attempts, no medical therapies have been shown to improve the natural history of AHF. In addition, the place of diagnosis of AHF is increasingly made in the outpatient setting. In this view, AHF is a moving target, and from recent registry data and from clinical trials, 5 critical lessons regarding the syndrome of AHF emerge: (1) The period of clinical instability preceding AHF may be much longer than previously thought. (2) Refinement of tools used to aid the early and accurate diagnosis of AHF will impact patient outcomes. (3) Standard supportive care of patients with AHF includes early use of diuretics with frequent reassessment in nearly all patients and supplemental vasodilators and oxygen therapy in selected cases. (4) Patients who survive presentation of AHF continue to suffer high rates of re-presentation, death, and rehospitalization following discharge from either hospital or emergency department. (5) Interventions shown to improve patient outcomes for AHF to date are related to process of care rather than new medications or devices. This report reviews the recent literature regarding the presentation, diagnosis, management, and prognosis of AHF. Areas of future research priority are indicated and guidelines for improving treatment are provided. AHF is an important clinical area that has not been as intensively studied as chronic HF; it presents both important needs and exciting opportunities for research and innovation. L'insuffisance cardiaque aiguë (ICA) affecte presque tous les Canadiens avec une insuffisance cardiaque (IC) au moins une fois. Malgré plusieurs tentatives, aucun traitement médical n'a démontré une amélioration de l'histoire naturelle de l'ICA. De plus, le diagnostic de l'ICA est de plus en plus présent dans le contexte de clinique externe. Dans cette optique, l'ICA est une cible mobile, et selon de récentes données provenant de registres et d'essais cliniques, 5 leçons essentielles sur le syndrome de l'ICA ressortent : (1) La période d'instabilité clinique précédant l'ICA peut être beaucoup plus longue que ce que l'on avait pensé préalablement. (2) Le raffinement des outils utilisés pour contribuer à un diagnostic précoce et précis de l'ICA se répercutera sur l'évolution de l'état de santé du patient. (3) Les traitements symptomatiques standards pour les patients avec une ICA incluent l'utilisation précoce de diurétiques et des réévaluations fréquentes chez presque tous les patients, en plus de la prise de vasodilatateurs et de l'administration d'une oxygénothérapie chez des patients sélectionnés. (4) Les patients qui survivent à une ICA ont un taux élevé de récidive, de mort et de réhospitalisation après le congé de l'hôpital ou du service des urgences. (5) À ce jour, les interventions qui ont montré une amélioration de l'évolution de l'état de santé du patient dans l'ICA sont reliées au processus de soins plutôt qu'à de nouveaux médicaments ou dispositifs. Ce rapport revoit la littérature récente concernant le tableau clinique, le diagnostic, la gestion et le pronostic de l'insuffisance cardiaque aiguë (ICA). Les champs prioritaires de la recherche sont donnés et les lignes directrices pour améliorer le traitement sont fournies. L'ICA est un champ clinique important qui n'a pas été aussi intensivement étudié que l'IC, les deux présentant des besoins importants et des opportunités passionnantes pour la recherche et l'innovation.
