Cette observation est l’illustration d’un cas de néphropathie uratique révélée par une goutte polyarticulaire tophacée. Un homme de 77 ans, hypertendu connu, est adressé en néphrologie pour insuffisance rénale à 561 μmol/L de créatinine sérique dans un contexte de polyarthralgies avec des nodules au niveau des coudes. Une néphropathie uratique chronique était évoquée devant l’hyperuricémie, l’uraturie, le syndrome inflammatoire, l’hypotrophie rénale bilatérale à l’échographie et la confirmation radiologique et histologique des tophi. L’évolution à j60 était marquée par une tendance à la normalisation de l’uricémie et une nette amélioration de la fonction rénale après traitement par du fébuxostat 80 mg/j en complément des mesures hygiénodiététiques. La plupart des données actuelles suggèrent que la thérapie de réduction de l’acide urique peut ralentir la progression de la maladie rénale chronique, bien que le mécanisme moléculaire de la néphropathie goutteuse reste à mieux comprendre. This observation is an illustration of a case of uratic nephropathy revealed by tophaceous polyarticular gout. A 77-year-old man with known hypertension was referred to nephrology unit for renal impairment with a 561 μmol/L serum creatinine level in a context of polyarthralgia with nodules on the elbows. Chronic uratic nephropathy was suspected on the basis of hyperuricaemia, uraturia, inflammatory syndrome, bilateral renal hypotrophy on ultrasound and radiological and histological confirmation of the tophi. The evolution at D60 was characterised by a tendency towards normalisation of the uricemia and a clear improvement in renal function after treatment with febuxostat 80 mg/d in addition to hygiene and dietary measures. Most of the current knowledge available suggests that uric acid reduction therapy can slow the progression of chronic kidney disease, although the molecular mechanism of gouty nephropathy needs to be better understood.
