Évaluation du handgrip préradiochimiothérapie des patients présentant un cancer des voies aérodigestives supérieures

La mise à jour des critères de diagnostic de la dénutrition de l’adulte a introduit l’évaluation de la quantité de masse musculaire et/ou de la fonction musculaire. La dénutrition étant un facteur d’interruption de traitement, en cas de radiochimiothérapie des cancers des VADS, nous avons évalué l’association entre la fonction musculaire préthérapeutique selon le handgrip test et les interruptions de traitement. Étude ancillaire d’une cohorte observationnelle, monocentrique, prospective évaluant l’intérêt d’une prise en charge systématique, globale et précoce en soins de support chez les patients présentant un cancer des voies aérodigestives supérieures (VADS) avant radiochimiothérapie concomitante. Étaient inclus tous les patients pour qui une radiochimiothérapie concomitante était envisagée pour le traitement d’un cancer des VADS et ayant eu une évaluation de la force de préhension préthérapeutique par le handgrip test. Trente-neuf patients ont été inclus de juillet 2019 à février 2020. Neuf patients étaient dénutris, dont six présentaient une force de préhension pathologique. L’évaluation du handgrip a permis de reclasser trois patients dénutris. Les interruptions de la radiothérapie étaient significativement associées à la présence d’une force de préhension préthérapeutique pathologique ( p = 0,0359). Aucune association n’a été retrouvée pour une interruption de la chimiothérapie ( p = 1). L’utilisation du handgrip test, dans le parcours de soins des patients atteints de cancers des VADS, montre un intérêt dans l’évaluation préthérapeutique sur le choix des traitements oncologiques au vu des risques de toxicités induites pouvant entraîner une interruption de traitement. As undernutrition is a factor in the interruption of radio-chemotherapy treatments for head and neck cancer, we evaluated the association between pre-therapeutic muscle function according to handgrip strength and treatment interruptions. Were included patients for whom radio-chemotherapy concomitant was being considered for radical treatment of head and neck cancer and who had a pre-therapeutic handgrip test. Thirty-nine patients were included from July 2019 to February 2020. Nine patients were undernourished and three had pathological handgrip strength. Handgrip test permitted to reclassify three patients as undernourished. Radiotherapy discontinuation was significantly associated with a pathological pre-therapeutic handgrip strength ( P = 0.0359). No association was found for discontinuation of chemotherapy ( P = 1). Pre-therapeutic handgrip test for patients with head and neck cancers is of interest on the oncological treatments choice regarding toxicities risk that may lead to treatment discontinuation.
