Article Description
La chirurgie urologique fait partie des 3 chirurgies les plus fréquemment réalisées chez l’enfant en France. Elle se divise en 3 grandes entités (la chirurgie pénienne et inguinoscrotale, la chirurgie du bas appareil urinaire et celle du haut appareil urinaire) et concerne les enfants de la période néonatale à l’adolescence. Les chirurgies péniennes et inguinoscrotales sont dans la grande majorité de cas des interventions simples prises en charge en ambulatoire à l’inverse des chirurgies du bas et haut appareil urinaire. Les enfants concernés peuvent parfois être porteurs de malformations cardiaques, du système nerveux ou du système endocrinien, qui compliquent la prise en charge. À l’exception de la greffe rénale et de quelques chirurgies intéressant le nouveau-né, l’anesthésie générale des enfants bénéficiant d’une chirurgie urologique n’a pas de particularité. Les techniques d’anesthésie locorégionale qui incluent différents blocs, sont adaptées au contrôle de la douleur postopératoire et permettent une épargne morphinique. Urological surgery is one of the three surgeries most frequently performed in children. Surgical procedures include 3 major entities (penile and inguino-scrotal surgery, surgery of the lower urinary tract and surgery of the upper urinary tract). According to the procedure, newborns or teenagers may be concerned. Penile and inguino-scrotal surgeries are usually day-case procedures while upper and lower urinary tract procedures are commonly performed in in-hospital patients. Children may have cardiac, nervous system and/or endocrine congenital diseases that make it more complicated their perioperative management. Regional anesthesia and especially peripheral blocks suits the requirement of postoperative pain management in these settings, allowing a significant morphine sparing.
