Article Description
Connue sous le nom d'opération de Heller, la cardiomyotomie antérieure extramuqueuse est un geste palliatif qui lève l'obstacle représenté par le défaut de relaxation du sphincter inférieur de l'œsophage au cours de l'achalasie. Ses résultats sont supérieurs à ceux de la dilatation pneumatique. L'abord abdominal, pratiqué sous cœlioscopie, est actuellement le procédé de référence. La myotomie doit répondre à des critères précis de profondeur et d'étendue. Le risque majeur est représenté par la plaie muqueuse. La dissection du cardia peut être limitée, et la nécessité d'un montage antireflux est controversée.
