Article Description
Les relations entre maladies ostéoarticulaires et manifestations psychologiques ou psychiatriques sont complexes et font intervenir des phénomènes de différentes natures. Des événements de vie stressants et des caractéristiques psychologiques peuvent favoriser l'apparition et le maintien de certaines affections. Les maladies chroniques s'accompagnent habituellement de réactions psychologiques, mais peuvent se compliquer de véritables pathologies psychiatriques. Enfin, certains troubles rhumatologiques posent des questions diagnostiques et thérapeutiques. La polyarthrite rhumatoïde notamment est un modèle « classique » de maladie psychosomatique dont la survenue et l'évolution peuvent être influencées par des facteurs psychologiques ou des événements de vie. La douleur constitue le motif majeur de consultation en rhumatologie et, par son caractère subjectif, constitue un bon exemple d'intrication somatopsychique. Enfin, la spécificité clinique de la fibromyalgie et son lien avec les troubles psychiatriques font encore l'objet de discussion. Ces différents troubles illustrent les difficultés auxquelles le praticien peut être confronté et permettent de souligner l'importance d'une prise en charge globale du patient. Ces troubles et leurs relations avec les manifestations psychologiques sont abordés du point de vue de la comorbidité, de la compréhension psychosomatique et de l'attitude thérapeutique recommandée.
