En nutrition clinique, l'intérêt se porte actuellement sur les lipides structurés, triglycérides ou mélanges de triglycérides comprenant à la fois des acides gras à chaîne moyenne (compris entre C6:0 et C12:0) et des acides gras à chaîne longue (nombre d'atomes de carbone ≥ 14). La distribution de ces acides gras sur le squelette de la molécule de glycérol est faite au hasard ou au contraire — et c'est dans cette direction que l'on s'oriente de plus en plus — est chimiquement définie, l'acide gras à chaîne longue étant en position ns-2 protégée de l'activité des enzymes responsables de la digestion et de l'épuration des lipides exogènes. Le but des lipides structurés est de fournir, dans la même molécule de triglycéride, d'une part des acides gras à chaîne moyenne à vocation énergétique immédiate, et d'autre part des acides gras à chaîne longue à visée fonctionnelle. Le choix de ces derniers se fait suivant le but assigné aux lipides structurés : fournisseurs d'acides gras spécifiques ou d'acides gras précurseurs de tels ou tels eicosanoïdes ; propriété hypolipémiante, antithrombotique ou immunomodulatrice. Dans le cas de lipides structurés chimiquement définis, la biodisponibilité de l'acide gras en ns-2 est particulièrement élevée. Cette revue générale rappelle l'historique des lipides structurés, décrit leurs propriétés physicochimiques et fait le point des connaissances actuelles sur leur digestion, leur absorption, leur épuration, leur oxydation et leur influence sur le système immunitaire et le métabolisme protéique. In clinical nutrition, considerable interest is being paid to structured lipids, triglycerides and mixtures of triglycerides comprising both medium-lenth fatty acids (between C6:0 and C12:0) and long-chain fatty acids > 14 carbon atoms). The distribution of these fatty acids along the glycerol core is determined either by chance or — and this is an area of increasing interest — by chemical considerations ; the long-chain fatty acid in position ns-2 is protected from the activity of enzymes responsible for lipid digestion and elimination. The aim of developing structured lipids is to provide, within the same triglyceride molecule, both medium-length fatty acids for immediate energy requirements and long-chain fatty acids for functional requirements. The choice of the latter is based on the desired fate of the structured lipids (essential fatty-acid supply or fatty-acid precursors of various eicosanoids ; hypolipemiant, antithrombotic or immunomodulatory properties). In the case of chemically-defined structure lipids, the biovailability of the fatty acid at position ns-2 is particularity high. This general review recalls past developments with regard to structured lipids, describes their physical and chemical properties, and deals with present knowledge of their metabolism.
