Article Description
Les lambeaux libres constituent aujourd'hui un apport majeur et incontournable en chirurgie reconstructive cervicofaciale. Leurs principales indications résultent des chirurgies d'exérèse carcinologique de la tête et du cou, aboutissant à des pertes de substances complexes et souvent étendues. La maîtrise des différentes techniques de prélèvement des lambeaux et la réalisation des microanastomoses vasculaires nécessitent un apprentissage rigoureux. La surveillance postopératoire, particulièrement attentive à la vitalité du lambeau, constitue un des points clés de la réussite de ce type de chirurgie. Le lambeau fasciocutané antébrachial à pédicule radial (lambeau chinois) est le lambeau libre le plus utilisé actuellement pour les réparations des pertes de substances muqueuses de la cavité buccopharyngée et de l'hypopharynx. La réparation osseuse mandibulaire fait essentiellement appel au lambeau de péroné (fibula).
