Healthcare is in transition. The new model of healthcare promotes prevention rather than treatment, and healthcare delivery is shifting from hospital and clinic to home. Smart Health Home (SHH) integrates technologies of smart health and smart home to enable new modes of healthcare delivery. The framework for integrating resources to make SHH available for practical use is yet to be established. We propose a micro-meso-macro framework that combines individual (micro), community (meso), and population (macro) levels of healthcare system. Key technologies, including Internet of Things, blockchain, smart contracts, and federated learning, are combined to facilitate implementation of healthcare delivery at SHH. Early detection and fast responses to health emergencies are enabled by a set of rules that serve to direct and distribute information at the meso-level within each community, while implementation in terms of health and public health outcomes is at micro or macro level.
